Since seating is limited we ask that you RSVP as soon as it is convenient and not later than Tuesday, December 1st.  There are several ways to do this but all involve sending us the information indicated on the form below.  You can simply email the necessary information to us from our Contact Information page.  Or, you may email the information to us at stbarbarafw@gmail.com.  Or you may create a document using the form below, fill it out, and then email it to us as an attached document. (We may not be able to open the most recent Word formats.)
